Pre K & Kindergarten requirements
Please mail all medical requirements such as entrance physical exams and immunization records to my attention at Lincoln School 728 Westfield Ave. Westfield, NJ 07090. All incoming students for pre K and Kindergarten must have an entrance physical exam, health history and complete immunization record.

Health Screenings
Our school health screenings program for preschool and kindergarten children includes height, weight, blood pressure, hearing, vision and * color vision testing (*kindergarten students only). These tests are screening tests, not meant to be diagnostic. If your child fails a vision or hearing test you will be notified and a rescreening will be conducted within 2-3 weeks. If the rescreening test is failed a request for further examination will be made. A written referral form will be sent home. Please arrange to have your child seen by a doctor, have the form completed and return it as soon as possible.

Preventing the spread of germs
Wash hands with soap & water frequently and for the length of time it takes to sing the Happy Birthday song to prevent the spread of germs. Practice proper hand washing technique with your child; wet hands first with water then apply soap. Cover your cough and sneeze with your elbow or a tissue, then wash hands. Hand sanitizer is a good alternative when soap and water is not available.

Keep your child at home if symptoms of Influenza or Influenza-like-illness are present:
**Fever - Fatigue - Sore Throat - Cough - Body Aches - Stomach Symptoms**
